By enrolling in Shivaay Dance Academy, I acknowledge that:

Policy Acknowledgment

I have received a copy of Shivaay Dance Academy’s policies and I take full responsibility to read, understand, and follow all rules and policies outlined therein.

Refund Policy

I understand that Shivaay Dance Academy does not offer credits or refunds for classes missed due to:

• Holidays

• Vacations

• Weather-related cancellations

• Moving or schedule conflicts

Refunds or credits will only be considered in cases of documented injury.

Injury Waiver

I acknowledge that participation in dance classes carries a risk of physical injury.
I voluntarily assume all risks associated with myself or my child’s participation in any class, rehearsal, or event organized by Shivaay Dance Academy.

I understand that Shivaay Dance Academy is not liable for any injury that may occur during classes or performances.

💳 Payment Terms

• All payments made are exclusive of holiday weeks and are based on scheduled classes only.

• All tuition payments are set up as automatic payments and will be billed on the 1st of each month, based on the selected payment plan (monthly, quarterly, or yearly).

We understand that plans may change. If you need to drop or pause a class, please:
